With care management, a Caring Heart geriatric care manager will complete an intake assessment and evaluation of the senior in the convenience of their own home. The care manager assesses the individuals strengths and needs in relation to all faucets of life such as emotional well-being, health, and cognitive status. The care manager assesses informal support for the individual and the need for professional or formal support to fill in the gaps. Every senior is different and unique. One senior may need a little assistance once a week. Others may need 24 hour care. The care manager will provide these services and/or refer to the needed supportive senior resources in the community. The coordination and supervision of these services are managed by the Caring Heart care manager. Examples of supportive geriatric resources can include, but are not limited to the following: personal care services, home making services, home health services, home delivered meals, adult day services, live-in assistance, respite care services, medical equipment, transportation arrangement, escort to necessary medical and/or social appointments, home repair and maintenance, senior friendly home modifications, social and therapeutic activities, 24-hour personal emergency response systems, and assistance with bill paying
